In-Home Transition Services
The purpose of Aftercare Services is to help participants continue the process of "forward walking" which was begun while participating in ANASAZI and to prepare to overcome potential challenges which students and family members may encounter upon their return to the "wilderness of the world." ANASAZI offers the following Aftercare Services:
  1. Discharge Plan
    An aftercare plan is developed prior to completion of the trail program. The family's Shadow (therapist) will make an assessment of the YoungWalker's progress and needs, as well as the concerns and expectations of parents/caregivers and others interested in the well being of the YoungWalker (i.e., extended family, school administrators, law enforcement, therapists and other helping professionals, etc.). The discharge plan will include specific recommendations for meeting goals and sustaining change. The plan may also include recommendations for testing, outpatient counseling, long-term residential care, etc.
     
  2. Wrap Around
    While at ANASAZI, the YoungWalker and parents/caregivers may identify and invite those interested in the well being of the YoungWalker to participate in a "wrap around" team to provide support and accountability to the YoungWalker and family. One week prior to completion of the trail program, the family's Shadow will provide an outline of specific concerns and targeted goals for the YW and family. Parents/caregivers will be invited to share the concerns and goals to the team and help each member of the team identify objectives and strategies to help.
     
  3. For Fee Services
    ANASAZI provides fee-based services to families who desire further therapeutic intervention including outpatient therapy through individual, couples, group, or family therapy provided at the ANASAZI office facility or through telephone "coaching" sessions for those outside of the local area.
Additionally, parents/caregivers are welcome to participate in The Choice in Intervention seminars at the ANASAZI site for up to a year with no additional charge ($100 fee for replacement or new materials if necessary). Parents and YoungWalkers can also find support through active participation in Alumni Services.

ANASAZI offers comprehensive in-home transition services to families completing its 42-day program. The optional six-week course augments ANASAZI's existing aftercare offering with extensive assessment, planning, and parent coaching initiated by a transition specialist during a three-day home visit.

Administered in partnership with the transition management experts at Homeward Bound, the program is designed to help families apply principles learned at ANASAZI to create a home environment that enhances family unity and promotes a lasting and positive change in family behavior.

Services include:
  • Three-day in-home visit by a transition specialist who will help facilitate family discussion, establish structure, identify current issues, and move the family toward desired outcomes
  • Panoramic assessment of family background and dynamics, with input from the family's chosen community, program and educational professionals
  • Telecoaching, provided during scheduled sessions and based on the principles taught at ANASAZI and in Homeward Bound's proven transition management curriculum
  • Direct access to a transition specialist who will help the family, as needed, to find solutions to the problems of the moment
  • One-year subscription to the online Family Bridge parent resource library, with access to valuable family assessment and communication tools
The cost for the program is $7900, plus air fare required for the in-home visit by a family transition specialist.
